Protected Health Information (PHI) is additionally governed by HIPAA and our Notice of Privacy Practices, which is provided at intake.

- Information you provide: name, email, phone, date of birth, insurance information, intake forms, clinical information shared during sessions.
- Automatically collected: IP address, device/browser type, pages viewed, referring URL, timestamps via cookies and similar technologies.
- From third parties: insurance carriers (eligibility & claims), referring providers, and payment processors.
How We Use Information
- To provide, schedule, and bill for clinical services.
- To communicate about appointments, care, and administrative matters.
- To comply with legal, regulatory, and licensing obligations.
- To improve our website and services (aggregated, non-identifiable analytics).
HIPAA & Protected Health Information
We follow HIPAA Privacy and Security Rules. We use HIPAA-compliant telehealth and electronic health record platforms with Business Associate Agreements in place. Our full Notice of Privacy Practices is provided during intake and available on request.
Sharing
We do not sell your personal information. We share information only:

- With service providers (EHR, telehealth, billing, payment processing) under confidentiality and BAAs as applicable.

Clinical records are retained for the period required by Florida law and applicable professional regulations. Website data is retained only as long as needed for the purposes described above.
